Job summary

Bristol Hospice in Honolulu, HI is seeking a Clinical Supervisor to ensure compassionate hospice care and to supervise clinical personnel across the care team.

The role requires an RN license in good standing, prior hospice home care management experience, and strong communication skills. The position is full-time with a daytime shift and a salary range of $120,000 to $130,000 per year, reflecting market rates for Hawaii.

Qualifications

• A Registered Nurse license in good standing to practice professional nursing in the state.
• Must have previous experience in a hospice home care setting with two years of management or supervisory experience.
• Enthusiasm for providing the best possible care to patients and their families.
• Understanding of the hospice philosophy and issues of death and dying.
• Excellent observation, verbal, and written communication skills.
• A valid driver’s license.
• Current CPR certification.

Responsibilities

• Receive case referrals and review patient information to determine hospice/home care needs.
• Assign appropriate hospice personnel to referral case as needed.
• Confer with Medical Director and Attending Physician regarding eligibility for services.
• Review and evaluate each case through home visits, conferences, records review, and clinician services.
• Review patient diagnosis, prognosis, medications, procedures and clinical course.
• Guide clinicians to improve performance and delivery of quality home care services.
• Be available during operating hours to assist clinicians as appropriate.
• Conduct clinical record reviews and communicate findings to the Clinical Director and hospice personnel.
• Assist in screening and interviewing new employees and make recommendations for employment.
• Assist in orientation of new employees.
• Assist in planning and evaluating education programs.
• Assist in formulating and evaluating organization policies and goals.
• Comply with professional standards.
• Assess community health needs and develop plans to meet them.
• Participate in public relations and community activities.
• Promote customer service orientation to all hospice personnel.
• Participate in quality improvement teams as directed.
• Perform other duties as delegated by the Director of Patient Care Services.
• Additional duties as assigned.
