Job summary

Hospice of the Piedmont, Inc. is seeking a full-time, exempt RN Case Manager in North Carolina to deliver compassionate, quality end-of-life care.

The position is Monday–Friday, 8am–5pm (40 hours/week) with a valid NC RN license required. Essential duties include scheduling patients, documenting visits, developing and coordinating care plans, communicating with the Nurse Practitioner, supervising CNA services, contacting families about patient changes, and actively participating in the

Qualifications

• Graduate of accredited school of nursing; Associate degree required, BSN preferred.
• Current North Carolina RN license.
• One year of clinical nursing experience preferred; hospice experience desirable.
• Valid NC driver's license and transportation.
• Certification in Hospice & Palliative Care desirable.

Responsibilities

• Schedule patients.
• Document visits (assessments, interventions, updates).
• Develop and coordinate plan of care.
• Communicate with Nurse Practitioner.
• Supervise CNA services for assigned patients.
• Make calls to families to explain patient changes.
• Participate in the Interdisciplinary Group (IDG) meetings.
