Responsibilities include patient assessments, care plan development, coordination with interdisciplinary teams, family education, and thorough regulatory documentation. Some travel to Macomb and Oakland County is required.

Qualifications

• Valid Registered Nurse (RN) license.
• Previous hospice nursing experience required.
• Strong clinical assessment and care planning skills.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized care plans.
• Coordinate and communicate effectively with interdisciplinary teams, patients, and families.
• Monitor patient progress and adjust care plans as needed to ensure quality and compliance.
• Provide education and emotional support to patients and their families throughout the hospice journey.
• Ensure all documentation meets regulatory and company standards.
