Job summary
University Home Care Inc in Oakland, MI is seeking a dedicated Hospice Case Manager (RN) to provide compassionate end-of-life care and support to patients and families.
The role requires an active RN license and hospice experience, with travel across Macomb and Oakland County to coordinate care, educate families, and ensure regulatory compliance.

Qualifications
- Valid RN license and current licensure in Michigan.
- Hospice nursing experience required.

- Ability to work independently and as part of a multidisciplinary team.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Coordinate and communicate effectively with interdisciplinary teams, patients, and families.
- Monitor patient progress and adjust care plans as needed to ensure quality and compliance.
- Provide education and emotional support to patients and their families throughout the hospice journey.
- Ensure all documentation meets regulatory and company standards.
