Job summary

Optimal Care in Grand Rapids, MI is seeking a Hospice RN Case Manager to provide skilled nursing care and compassionate support to patients and families at the end of life. You will coordinate home-based care, build strong relationships with patients, families, physicians, and facilities, and operate with autonomy within an interdisciplinary team.

Full-time role with daily travel across Kent and surrounding counties.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• Minimum one (1) year experience as a nurse in a clinical care setting.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.

Responsibilities

• Build rapport with patients, families, physicians, facilities, and interdisciplinary team members.
• Coordinate comprehensive home-based care for your caseload in collaboration with the inter-disciplinary team.
• Address physical, emotional, and spiritual needs of those in your care.
