Inhouse Hospice Llc in Michigan is seeking an experienced Registered Nurse (RN) Case Manager to join our hospice team. You will manage a caseload of 12-15 patients, coordinate interdisciplinary care, and ensure comfort and dignity at end-of-life.

Qualifications

• Active RN license in Michigan is required.
• Minimum 1 year hospice or home health experience preferred.
• Strong clinical assessment and critical thinking skills.
