Job summary

Morning Star Hospice and Palliative Care invites an experienced RN to join as a Hospice Case Manager serving the Lebanon/Myerstown, PA area. You will coordinate care for patients at home, in facilities, or assisted living, collaborating with an interdisciplinary team to ensure comfort and dignity at end of life.

The role emphasizes accurate assessments, individualized care plans, medication management, and education for families and patients on disease processes and end-of-life options.

Qualifications

• Current and unrestricted RN license in Pennsylvania.
• Hospice or palliative care experience preferred (1+ year).
• Strong assessment and care planning skills.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care including medication management, pain control, symptom management, and emotional support.
• Act as liaison between patients, families, and the interdisciplinary care team to ensure coordinated care.
• Educate patients and families on disease processes, medications, and end-of-life care options.
• Monitor patient progress and adjust care plans as needed to ensure comfort and dignity.
• Document patient care and progress in compliance with hospice regulations and company policies.
• Participate in regular team meetings to discuss patient status and plan of care.
