To provide nursing care to terminally ill hospice patients as needed. To assist and support the family in home care and bereavement. To work as part of the hospice team in delivering care.

Qualifications
  • Educational/Degree: Graduate from an accredited school of nursing
  • Training/Licensure: Currently licensed as a registered nurse in the state
  • Knowledge/Skills/Abilities: Ability to work independently, make quick and accurate judgments, supervise others appropriately, respond to crises outside hospital settings, and adapt to diverse social, racial, cultural, and religious backgrounds. Completion of hospice training program.
  • Experience: Minimum 2 years as a registered nurse preferred; active patient contact within the past three years preferred.

Physical Requirements

Requires significant physical effort, including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature/humidity changes, and assisting in lifting/transferring patients up to 100 pounds. Must have functional sight and hearing or appropriate adaptive devices. Must provide evidence of annual TB testing and other required examinations.

Mental Requirements

Ability to work independently, assess data, make judgments, and act accordingly. Must be flexible, innovative, and possess good interpersonal skills. Capable of handling mental and emotional stress with emotional stability.

Working Conditions

Exposure to odors, blood, body fluids, hazardous materials, and adverse environmental conditions.

Transportation

Valid driver’s license, auto liability insurance, and reliable transportation required.

Essential Functions
  1. Assess and reassess nursing needs of hospice patients regularly.
  2. Provide dietary counseling and hospice nursing services, treatments, and preventive procedures.
  3. Initiate appropriate nursing procedures for patient safety and care.
  4. Observe and report any physical or emotional changes to the physician and team.
  5. Educate and counsel patients and families on care provision.
  6. Supervise and train nursing personnel.
  7. Develop and update care plans in collaboration with the interdisciplinary team.
  8. Perform treatments and symptom management as ordered.
  9. Participate in team conferences and maintain accurate records.
  10. Follow hospice policies, maintain confidentiality, and comply with HIPAA.
  11. Communicate any inability to meet patient needs to supervisors.
  12. Participate in on-call duties and maintain skills and knowledge.
  13. Coordinate care for patients in various residential settings.
  14. Organize work schedules efficiently.
  15. Adhere to infection control protocols.
  16. Assist with orientation, teaching, and training as needed.
  17. Perform other duties as assigned by the Director.
