Clinical Manager/Manager of Patient Services
REPORTING RELATIONSHIP:
Supervised by: Director
Positions Supervised: Hospice Staff
Interrelationships: Patients, family, IDG and other healthcare team members
JOB SUMMARY:
Responsible as a member of the interdisciplinary and clinical management teams to the Director for the implementation, delivery, coordination, and supervision of hospice services delivered by the respective team, consistent with hospice policies and procedures.
Qualifications:
- Educational/Degree: Graduate from an accredited school of nursing
- Training/Licensure: Currently licensed as a registered nurse to practice in the state.
- Knowledge/Skills/Abilities: Ability to work independently, make accurate and quick judgments, supervise others appropriately, respond to crises outside of a hospital setting, and adapt to different social, racial, cultural, and religious modes.
- Experience: Minimum 2 years of experience as a registered nurse, preferred. Active patient contact within the past three years, preferred.
JOB FACTORS:
- Physical Requirements: Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes, and maximal assistance in lifting/transferring a 100-pound patient. Must possess sight/hearing senses or use appropriate adaptive devices. Must provide evidence of annual TB test and other required examinations.
- Mental Requirements: Ability to work independently, make assessments and act accordingly, be flexible, innovative, and possess good interpersonal skills. Capable of coping with mental and emotional stress and demonstrating emotional stability.
- Working Conditions: Tolerance to exposure to odors, blood, body fluids, adverse environmental conditions, and hazardous materials.
Job Description – Manager of Patient Service
Transportation: Must have a valid driver’s license, auto liability insurance, and reliable transportation.
Essential Functions:
- Supervise planning, coordination, and delivery of interdisciplinary group interventions.
- Assign patients/families to appropriate staff.
- Lead and participate in interdisciplinary team meetings, care plan development, problem-solving, and team support.
- Ensure compliance with policies and procedures.
- Participate in management team activities, address service delivery issues, staff development, and policy suggestions.
- Maintain accurate, complete, and compliant medical records.
- Supervise support and clerical staff.
- Oversee clinical supervision of nursing personnel, hospice aides, and homemakers.
- Conduct supervisory visits for clinical observation.
- Assist in performance evaluations and staff development.
- Identify staff education needs and develop in-service programs.
- Participate in new personnel orientation.
- Perform specific assignments as directed.
- Reassess nursing needs of patients regularly.
- Provide nursing services, treatments, and preventive procedures.
- Educate and counsel patients and families.
- Perform nursing procedures as ordered by physicians.
- Maintain confidentiality per HIPAA regulations.
- Communicate any issues to the Director.
- Participate in on-call coverage.
- Maintain skills and knowledge.
- Participate in QAPI programs.
- Perform other duties as assigned.
