As a Hospice RN / RN Case Manager, you will:

1. Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary control and professional management of each patient.
2. Act as the primary liaison between the physician, patient/family, and hospice team.
3. Provide direct patient care services to hospice patients and supportive care to their families, managing a reasonable caseload while building trusting and compassionate relationships.
4. Empower caregivers through education on care for their loved ones.
5. Manage and collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
6. Evaluate patient and family needs and assist in developing and executing individualized care plans.
7. Maintain all assigned clinical records accurately.
8. Participate in quality assessment and performance improvement programs.

About You

Hospice Registered Nurse / RN Case Manager Requirements:

• Compassionate, kind, and empathetic towards patients and families.
• Comfortable providing care wherever the patient calls “home”.
• Flexible and composed in supporting others during crises.
• RN license in the requested state (or eligibility to obtain).
• At least 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology is preferred.
• Additional experience in hospital settings such as Staff RN, Med Surg, PACU, ER, or critical care is advantageous.
• Valid driver’s license and auto insurance.
• Current CPR certification.
