Job summary

The Staff Pad seeks an experienced RN Case Manager to deliver compassionate hospice care in patients' homes in the Dallas area. You will coordinate individualized care plans with patients, families, and the interdisciplinary team, serving as an advocate for high-quality care throughout the hospice journey.

Reporting to the Managing Director, this role requires autonomy, strong clinical judgment, and dedication to professional growth.

Qualifications

  • RN license in Texas is required.
  • Current CPR certification.
  • 1–3 years of clinical nursing experience; Hospice experience preferred.

Responsibilities

  • Perform comprehensive patient assessments and develop individualized care plans.
  • Deliver skilled nursing care in patients' homes per hospice standards.
  • Coordinate care with patients, families, physicians, and the IDT.
  • Supervise hospice aides and LVNs to ensure continuity of care.
  • Maintain accurate, compliant clinical documentation.
  • Complete patient admissions and participate in on-call rotation.
  • Manage patient schedules and changing care needs.
  • Promote infection control and patient advocacy.
  • Assist with training and mentoring new staff.

Reporting to the Managing Director – Hospice, the RNCase Manager, provides skilled nursing care to hospice patients in their homeswhile coordinating individualized care plans with patients, families, and theinterdisciplinary team (IDT). You'll serve as a trusted advocate, ensuringcompassionate, high-quality care throughout the hospice journey.

What You’ll Do
Key Responsibilities
  • Perform comprehensive patientassessments and develop individualized care plans
  • Deliver skilled nursing care inpatients homes in accordance with hospice standards
  • Coordinate care with patients,families, physicians, and the IDT
  • Supervise Hospice Aides and LVNs toensure continuity of care
  • Maintainaccurate, compliant clinical documentation
  • Complete patient admissions andparticipate in PRN on-call rotation
  • Manage patient schedules andprioritize changing care needs
  • Promote infection control, patientadvocacy, and agency values
  • Assist with training and mentoringnew clinical staff as needed
  • Commit to ongoing professional growth and clinicalexcellence through continuous education
What We Are Looking For
Qualifications
  • Graduate of an accredited Nursingprogram (Diploma, ADN, or BSN)
  • Active Texas RN license required; CurrentCPR certification
  • 1-3 years of clinical nursingexperience required; Hospice, ER, or OR experience preferred
  • Reliable transportation, validTexas driver's license, and current auto insurance
  • Must pass background check andprovide required TB and Hepatitis documentation
Additional Requirements
  • Ability to stand, walk, safely movepatients, lift up to 50 lbs. and travel locally
  • Ability to work effectively inhigh-pressure, fast-paced environments
  • Strong clinical judgment and quickdecision-making skills
  • Ability to meet the individualizedpsychosocial needs of patients and families
