Job summary

LeoForce in The Bronx NY is seeking an RN Hospice Business Development Care Planner to evaluate and establish a plan of care for patients referred for hospice services and coordinate admissions from hospital discharge.

You will work on-site at a major medical center in the Bronx with a Monday–Friday 9am–5pm schedule (no weekends). You will build and maintain strong relationships with physicians, social workers, case managers, and discharge planners to ensure seamless care.

Responsibilities

• Evaluates and establishes a plan of care for patients referred for hospice services.
• Facilitates admission of referrals from hospital to a hospice program that meets patient and family needs.
• Develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.
