Job summary

Alabama Hospice of the Shoals, part of the LHC Group within the Optum family, seeks a Patient Care Manager - Hospice to oversee nursing-driven patient care in home settings and ensure compliance with clinical standards. This role supports directors in coordinating care delivery and staff education.

The ideal candidate will have an unrestricted RN license, reliable transportation, and a track record of independent, high-quality care.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Assist Executive Administrator/Executive Director/Director of Clinical Operations with patient care activities to ensure safe and efficient care.
• Ensure compliance with hospice policies, procedures, and regulatory requirements.
• Promote quality of care and safety for patients and staff; engage in problem resolution and investigations.
• Provide in-services to staff on regulatory issues and agency policy; address educational needs.
• Ability to function in diverse home environments and with varied client needs.
