Job summary

CommonSpirit Health through CHI Health at Home is seeking a Hospice Registered Nurse to deliver compassionate in-home hospice care. You will assess patients, implement care plans, and coordinate with a multidisciplinary team to optimize comfort and outcomes.

The role requires strong critical thinking, patient-centered care, and the ability to educate families. A valid RN license, CPR/BLS, and 1+ year of nursing experience are required.

Qualifications

Responsibilities

• Deliver individualized, high-quality hospice care in patients' homes.

• Educate patients and families, providing emotional support and guidance.
• Mentor and guide LPNs, LVNs, and Home Health Aides.

*No After Hours On-Call Requirements*

• Be a clinical leader. Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care.
• Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.
• Collaborate and coordinate. Work seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care.
• Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge AI-powered clinical documentation and best practices to optimize care delivery and maintain precise documentation, promoting work-life balance and more patient interaction.

• Job Requirements*Required
• Graduate of an accredited school of nursing.
• Must have an active RN license in the state served.
• Minimum of 1 year of nursing experience required
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

*Preferred*

• Two years of hospital / hospice / homecare RN nursing experience preferred.
