Corewell Health's Hospice West team in Grand Rapids seeks a dedicated Hospice Nurse Case Manager to coordinate end-of-life care. You will assess patients, develop care plans, and work with an interdisciplinary team to ensure compassionate, compliant, high-quality hospice services.
The role involves home, facility, and hospital visits, educating families, and maintaining precise documentation. Full-time evening shift with 36 hours per week.
Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. Under the direct supervision of the Nursing Manager, the Hospice Nurse Case Manager is responsible for coordinating and delivering compassionate end-of-life care to patients and their families. This role involves assessing patient needs, developing individualized care plans and collaborating with an interdisciplinary team to ensure the highest quality of hospice care.
