We are now hiring an RN for Home Visits in the Winchester, KY area.

This is a PRN position.

Requirements:

• Must have experience with PEDS Trach/Vent Cases
• One Patient at a Time
• Full-time RNs (32+ hours) are eligible for PTO, Health Benefits, and 401k.

Specific Job Duties/Responsibilities:

• Provide high-quality clinical services within scope of practice and infection control standards, following the plan of care, and in coordination with the care team from admit to discharge.
• Complete clinical nursing assessments as per federal and state requirements and payer guidelines.
• Ensure patient eligibility and medical necessity for services.
• Develop and revise individualized care plans in collaboration with community providers.
• Ensure care plans meet patient needs, with revisions as needed and physician approval.

Experience Desired:

• Minimum of one year experience as an RN preferred.

License Requirements:

• Current RN licensure in practice state.
• Valid driver’s license, vehicle insurance, dependable transportation.
• Current CPR certification.

State Specific Requirements (KY):

• At least one year of clinical RN experience, which may be waived if recent LPN experience.
• RN licensure must have no restrictions.
