This is a PRN position.

• Must have experience with PEDS Trach/Vent Cases
• One Patient at a Time
• Flexible Scheduling
• RNs working full-time (32+ hours) will be eligible for PTO, Health Benefits, and 401k.

Responsibilities

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and helps maintain continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, case managers, and community resources.

Education and Experience

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within the scope of practice and infection control standards, following the care plan and coordinating with the care team from admission to discharge. Completes clinical nursing assessments as required by federal, state, and payer requirements.
• Ensures patient eligibility and medical necessity for services according to payer source and agency policy.
• Develops and revises individualized care and service plans in accordance with federal and state requirements, collaborating with community providers.
• Ensures the plan of care's frequency and duration meet patient needs and revises the plan with physician approval as needed.

Experience Desired

• Minimum of one year experience as an RN preferred.

License Requirements

• Current RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation.
• Current CPR certification required.

State Specific Requirements

• KY: At least one year of clinical experience as an RN; this may be waived if the candidate has recent clinical experience as an LPN.
• RN licensure must have no restrictions.
