Job summary

Visiting Nurse Association Health Group, through the Children & Family Health Institute (CFHI), is hiring a Home Visitor RN in Monmouth County, New Jersey. You will deliver Nurse‑Family Partnership nursing visits to first‑time mothers, support healthy pregnancies, child development, and family well‑being in home settings.

The role requires an Associate’s degree in Nursing, 2–3 years of maternal/child health nursing experience, strong communication and organizational skills, and computer

Qualifications

• Associate’s degree in Nursing required.
• 2–3 years of nursing experience; home visiting or maternal/child health experience preferred.
• Proficiency in computer use and excellent verbal and written communication; strong organizational skills.
• Bilingual in English and Spanish preferred.

Responsibilities

• Conduct in‑home visits per the NFP model and manage a caseload of up to 25 clients.
• Assess and address the health and developmental needs of women and their families.
• Offer counseling and instruction to help families achieve targeted health and life goals.
• Build and maintain community relationships to facilitate referrals and resource connections.
• Attend weekly team meetings, case conferences, and clinical supervision sessions; mentor and precept new hires as needed.
• Ensure compliance with infection control and safety policies.
