VNA Health Group’s Children & Family Health Institute in Somerset County, New Jersey is seeking a compassionate Registered Nurse to join the Nurse-Family Partnership as a HomeVisitorRN. You will deliver in-home nursing care and guidance to first-time mothers and families, helping improve pregnancy outcomes and child development.

Responsibilities include managing a caseload of up to 25 clients, conducting assessments, counseling, connecting families with resources, and mentoring new hires.

Qualifications

• Associate's degree in Nursing
• 2-3 years of nursing experience; homevisiting or maternal/child health experience preferred
• Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities
• Bilingual (English and Spanish-speaking) required
• Commitment to CFHI’s mission and the ability to build trusting relationships with clients and colleagues

As aHomeVisitorRN, you will:

• Conduct in-homevisits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients

• Assess and address the health and developmental needs of women and their families

• Offer counseling and instruction to help families achieve targeted health and life goals

• Build and maintain strong community relationships to facilitate client referrals and resource connections

• Attend weekly team meetings, case conferences, and clinical supervision sessions

• Mentor and precept new hires and support student nurses duringhomevisits as needed

• Ensure compliance with agency infection control and safety policies

• Maintain professionalism in adherence to organizational policies on absenteeism and appearance

Qualifications

• Associate's degree in Nursing required

• Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities

This position is covered by a collective bargaining agreement and is represented by a Union.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $42.00 to USD $50.00.
