VNA Health Group’s Children & Family Health Institute in Somerset County, New Jersey is seeking a compassionate Registered Nurse to join the Nurse-Family Partnership as a HomeVisitorRN. The Children and Family Health Instituteis a charitable, nonprofit program of VNA Health Group focused on improving the health and well-being of children and families through accessible, community-based care. CFHI delivers high-impact programs including nutrition support, preventive screenings, vaccinations, and inclusive health services through community health centers and mobile outreach. Our goal is to strengthen scale and stability while advancing a mission that creates meaningful impact in the community.
Conduct in-homevisits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients
Assess and address the health and developmental needs of women and their families
Offer counseling and instruction to help families achieve targeted health and life goals
Build and maintain strong community relationships to facilitate client referrals and resource connections
Attend weekly team meetings, case conferences, and clinical supervision sessions
Mentor and precept new hires and support student nurses duringhomevisits as needed
Ensure compliance with agency infection control and safety policies
Maintain professionalism in adherence to organizational policies on absenteeism and appearance
Associate's degree in Nursing required
2-3 years of relevant nursing experience;homevisiting or maternal/child health experience preferred
Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities
Bilingual (English and Spanish-speaking) required
A commitment to CFHI’s mission and the ability to build trusting relationships with clients and colleagues
This position is covered by a collective bargaining agreement and is represented by a Union.
In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $42.00 to USD $50.00.
