Job summary

A community health organization in Somerset County, New Jersey, seeks a compassionate Home Visitor RN to deliver care through the Nurse-Family Partnership program. This role involves conducting in-home visits to assess families' needs and guide them towards healthier outcomes. The ideal candidate should possess a BSN, relevant nursing experience, and be bilingual in English and Spanish. Qualifications

• 2-3 years of relevant nursing experience; home visiting or maternal/child health experience preferred.
• Commitment to CFHI’s mission and ability to build trusting relationships with clients and colleagues.

Responsibilities

• Conduct in-home visits to provide nursing services aligned with the NFP model.
• Assess and address the health and developmental needs of women and their families.
• Offer counseling and instruction to help families achieve targeted health and life goals.
