Job summary

VNA Health Group is looking for a compassionate Home Visitor Registered Nurse (RN) to support first-time mothers and their families through the Nurse‑Family Partnership program in New Jersey. The role involves conducting in-home visits, assessing family health needs, offering counseling, and building community relationships. Candidates should have an Associate’s degree in Nursing, relevant experience, and bilingual proficiency in English and Spanish. Qualifications

• 2–3 years relevant nursing experience, preferably in home-visiting or maternal/child health.
• Proficiency in computer use is essential.
• Commitment to CFHI's mission and ability to build trusting relationships.

Responsibilities

• Conduct in-home visits providing nursing services aligned with NFP guidelines.
• Assess and address health and developmental needs of families.
• Offer counseling to help families achieve health goals.
• Build strong community relationships for client referrals.
• Attend team meetings and mentor new hires.
