Job summary

Option Care Health in Greenwood, SC seeks an experienced Registered Nurse for on-site home IV therapy coordination and patient education. You will assess candidates for home infusion, train patients and caregivers, and collaborate with pharmacists and care teams to ensure safe therapy. Strong clinical, communication, and organizational skills are essential, with SC licensure and drivers license required.

Qualifications

• Active South Carolina RN license
• Current South Carolina driver’s license
• 2+ years of experience, preferably in home IV therapies
• Excellent clinical, communication and organizational skills

Responsibilities

• Perform initial patient assessment for home IV therapy suitability
• Coordinate with pharmacy and IP team to develop initial treatment plan
• Educate patients and caregivers on home IV therapy and device operation
• Train patient/caregiver on administration techniques and equipment
• Monitor patient progress during hospitalization and home therapy
• Coordinate discharge planning and collaboration with case managers and hospitals
• Document all coordination activities and maintain updated care plans
• Maintain professional relationship with physicians and case managers at assigned accounts
