Job summary

Magenta Care LLC seeks a skilled Registered Nurse for direct patient care in home health. You will evaluate patient needs, implement plans, and educate families on infusion therapy while coordinating with physicians and the care team.

Role requires active RN licensure in WA, CPR/BLS, and IV proficiency, with 1+ year home health experience. BSN preferred, CRNI/IgCN certifications valued, and valid driver’s license with reliable transportation.

Qualifications

Responsibilities

• Provide direct patient care within the RN practice act.
• Assess, diagnose, plan, implement and evaluate patient care with patient/family involvement.
• Collaborate with physicians and interdisciplinary team to develop care plans.
• Facilitate start of care through the admission process.
• Act as patient advocate to ensure rights, safety and preferences.
• Document changes promptly to the team, Director of Clinical Services, and physician.
• Educate patients and families on infusion therapy management and self-care.

Responsibilities

Provide direct patient care, practicing within the scope of the registered nurse practice act. Continuously assess, use nursing diagnosis, implement, plan, and evaluate patient care, actively involving patients and their families in the process. Collaborate with physicians and other interdisciplinary team members to develop a comprehensive plan of care that addresses the unique needs of each patient and their family. Facilitate patient start of care through the admission process. Act as a patient advocate, ensuring their rights, preferences, and safety are upheld. Document and effectively communicate any changes in the patient's condition promptly to team members, the Director of Clinical Services, and the physician. Provide nursing care to home health patients, adhering to established standards of care and following agency policies and procedures. Educate patients and their families on infusion therapy management, potential side effects, and self-care practices

Requirements

• CRNI (Certified Registered Nurse Infusion) or IgCN (Immunoglobulin Certified Nurse) certification preferred.
• Valid Registered Nurse (RN) license in the state..
• Bachelor of Science in Nursing (BSN) preferred, but not required.
• Minimum of 1 year of recent home health RN experience preferred.
• Excellent IV insertion skills.
• Healthcare provider CPR and Basic Life Support (BLS) certification.
• Current state Driver's license with proof of insurability.
• Reliable transportation.
