Qualifications

• Licensed as a Registered Nurse (RN) in the state of employment.
• Preferred: Minimum of one year of clinical experience.
• Valid state driver’s license and automobile liability insurance.

Responsibilities

• Perform skilled nursing visits and complete coordination of client care.
• Serve as point of contact for all disciplines involved in patient care.
• Oversee the frequency of visits for the episode.
• Consult with the physician and the office as needed.
• Perform accurate OASIS collection.
• Ensure lab values are reported to the physician in a timely fashion.
