Qualifications

Responsibilities
- Perform skilled nursing visits and coordinate client care.
- Act as point of contact for all disciplines involved in patient care.
- Oversee frequency of visits for the episode.
- Consult with the physician and office on patient care details.
- Accurately collect OASIS data, ensuring medication profiles remain current.
- Ensure lab values are reported to the physician in a timely manner.
- Attend weekly case conferences and monthly case manager meetings.
