Job summary

Enhabit Home Health & Hospice seeks a registered nurse to coordinate patient care in a home health setting. The role requires RN licensure and 2+ years of nursing experience, including home health or hospice exposure.

You will work from an office-based setting in Florida, interfacing with physicians, patients, caregivers, and staff to ensure high-quality care. Applicants must meet regulatory requirements and demonstrate strong clinical and technological proficiency, including mobile device use,

Qualifications

• RN license required.
• Graduate of an approved school of professional nursing.
• Licensed in the state or compact if applicable.
• At least two years of nursing experience.
• At least one year in home health or hospice.
• Knowledge of federal, state and local home care regulations.
• Basic technology skills, including mobile devices.

Responsibilities

• Coordinate home health services for the patient per standards and procedures.
• Review and approve patient information during start of care, recertification, resumption, or evaluation.
• Facilitate relationships among physicians, referral sources, patients, caregivers and staff.
