Compassionate Care Home Health in California seeks an RN Clinical Supervisor for Tulare County and surrounding territories. The role oversees nursing staff, ensures adherence to care plans, and maintains regulatory compliance across supervised caseloads.

You will lead case management activities, coordinate interdisciplinary care, and ensure timely visits and durable discharge plans while maintaining high-quality patient care and team performance.

Qualifikationen

• Active, unrestricted California RN license.
• Minimum 3 years home health experience.
• Minimum 1 year in a supervisory, charge, or clinical‑lead role.
• Current BLS certification; valid CA driver’s license and auto insurance.

Aufgaben

• Ongoing audit of supervised caseloads and chart review.
• LVN supervisory visits and competency validation.
• Timeliness oversight: SOC/ROC within 48 hours of referral or return home.
• Pop‑up and joint visits: direct observation of practice.
• QAPI participation: bring case‑management data to the committee.
• Weekly reporting to DPCS and staff accountability actions.
