Job summary

Azza HealthCare Agency is seeking aRegistered Nurse to provide direct patient care in a home health setting. You will assess preferences and cultural needs, develop and document personalized care plans, and collaborate with families and the healthcare team.

Responsibilities include monitoring patient condition, updating plans, and ensuring evidence-based practice and quality outcomes. Must meet licensure requirements and be willing to cover multiple regions.

Qualifications

• RN license and driver's license are required.
• Minimum nursing diploma or ADN; 2+ years nursing experience; bachelor's degree preferred.
• Must be willing to provide coverage across multiple regions and follow safety protocols.
