Job summary

CHRISTUS Health is seeking an RN Navigator in Population Health to coordinate patient care across the healthcare continuum within CHRISTUS Health ministries. You will develop individualized care plans, promote preventive care, and ensure timely interventions to improve population health outcomes.

The role requires strong clinical assessment, exceptional communication, and the ability to work both independently and as part of a team.

Qualifications

• Bachelor's Degree in Nursing preferred.
• RN license in state of employment or compact.
• 3 years of clinical experience required.
• 2 years of case management experience required.
• Experience in a primary care value-based care organization is required.
• Knowledge of population health management principles is required.

Responsibilities

• Care coordination for complex and chronic conditions and discharge planning support.
• Transitions of care: educate patients, ensure follow-up and adherence.
• Perform comprehensive patient assessments and identify barriers to care.
• Develop and implement individualized care plans aligned with guidelines and patient preferences.
• Advocate for patients and help navigate the healthcare system.
• Collaborate with providers, social workers, and community resources for holistic care.
• Monitor and report significant changes to PCPs and specialists.
• Document interactions, plans, and outcomes in the EHR.
• Participate in quality improvement initiatives to improve patient care and outcomes.
• Ensure compliance with regulatory requirements and organizational policies.
