Elara Caring is hiring a Registered Nurse Home Health to join our elite team delivering Right Care at the Right Time in patients' homes across Texas and beyond. You will assess, plan, and provide nursing care, monitor patient conditions, educate families, and work with the care team to adjust plans as needed.

This role requires an active RN license, 1+ year of clinical experience, and the ability to travel between patient homes.

Qualifications

• Current State license as a Registered Nurse (RN).
• Graduate of an accredited nursing program (Certificate, Diploma, Associates or Baccalaureate).
• 1+ year nursing experience in a clinical care setting.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient privacy per HIPAA.
• Providing nursing care per plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on care and safety.
• Taking action when patient conditions change.
• Helping ensure quality and safe patient care.
