Elara Caring is seeking a Registered Nurse Home Health to join our team and provide compassionate, high-quality care to patients in their homes. You will conduct initial and ongoing assessments, follow physician orders, and adapt care plans to changing patient conditions, all while safeguarding privacy and maintaining HIPAA compliance.

The role requires an active RN license, at least 1 year of clinical nursing experience, and a willingness to travel between patient homes.

Qualifikationen

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.

• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.
