Responsibilities

• Complete initial and ongoing assessments for home health and/or hospice needs.
• Develop patient-focused care plans with goals based on nursing diagnoses.
• Provide compassionate care in patients' homes.
• Act as Case Manager and coordinate care for an assigned caseload.
• Collaborate with interdisciplinary team members.
• Communicate with physicians and obtain orders as required.
