Job summary

Elite Home Health & Hospice in Lewiston, Idaho is seeking a full-time Physical Therapist to join a high-performing home-health and hospice team. The role offers relocation-friendly opportunities, clinical autonomy, and collaboration with a respected organization in the region.

Qualifications

Current Idaho Physical Therapist license (or ability to obtain) within state requirements.
One year of PT experience preferred; home-health experience a plus.
Strong communication, assessment, and clinical reasoning skills.
Ability to work independently in a field-based environment.
Reliable transportation and valid driver’s license.

Responsibilities

Perform comprehensive physical therapy evaluations in patient homes.
Develop individualized treatment plans focused on functional mobility, safety, and independence.
Provide gait training, strengthening, balance work, and home exercise program development.
Conduct home-safety assessments and recommend DME or environmental modifications.
Manage a field-based caseload with strong organizational efficiency.
Collaborate with interdisciplinary team for coordinated, patient-centered care.
Maintain accurate documentation in line with standards.
Support safe transitions from hospitals, SNFs, and outpatient settings.
Schedule full-time daytime work Monday–Friday.
Balance caseload with strong team support.
