Job summary

CHI is seeking a Home Health Nurse to bridge hospital care with the comfort of patients' homes, delivering high‑acuity nursing across diverse conditions and ages.

You will perform initial assessments, create and revise care plans, and educate patients and families while documenting meticulously to support ongoing wellness and safety in home settings. A proactive, independent clinician is essential.

Qualifications

Graduate of National League for Nursing accredited school of nursing.
Experience in public health or home health nursing.
Registered Nurse: ND license.
Basic Life Support – CPR.
Driver's License: ND.
Auto Insurance.

Responsibilities

Completes an initial assessment of client and family to determine home health needs.
Provides a complete physical assessment and history of current and previous illness.
Initiates the plan of care and makes revisions as client status changes.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan with goals based on nursing diagnosis and involves client and family.
Initiates appropriate preventive and rehabilitative nursing procedures.
