Job summary

CommonSpirit Health is seeking a Home Health Nurse to bridge hospital care with the comfort of home, delivering high-acuity nursing care in patients' homes across diverse conditions. You will manage complex wound care, infusions, education, and documentation with an independent practice mindset.

Qualifications

• Graduate of NLN-accredited nursing program.
• Experience in public health or home health nursing.
• ND RN license.
• BLS/CPR certification.
• Driver's license and auto insurance.

Responsibilities

• Perform initial patient/family assessment to determine home health needs.
• Develop and revise care plans with goals based on nursing diagnoses.
• Administer medications and treatments as prescribed.
• Provide patient and family education and counseling.
• Ensure meticulous documentation of care and patient status.

Job Summary and Responsibilities

As our Home Health Nurse, you will be a pivotal healthcare hero, bridging hospital care with the comfort of home, empowering patients to heal in their own environment.

Every day, you will deliver personalized, high-acuity nursing care for a diverse range of conditions and ages directly in patients' homes. From complex wound care and infusions to vital patient education and meticulous documentation, you'll manage your dynamic schedule to provide comprehensive, life-changing support.

• Completes an initial assessment of client and family to determine home health needs. Provides a complete physical assessment and history of current and previous illness. Regularly re-evaluates client nursing needs.
• Initiates the plan of care and makes necessary revisions as client status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the client and family in meeting nursing and related needs.

Job Requirements

Required

• Graduate of National League for Nursing accredited school of nursing.
• Experience in the area of public health or home health nursing
• Registered Nurse: ND
• Basic Life Support - CPR
• Driver's License
• Auto Insurance

Pay Range

$39.18 - $58.28 /hour
