CommonSpirit Health at Home is seeking a dedicated Home Health RN to provide one-on-one, compassionate nursing care in patients' homes. You will assess, plan, and implement individualized care while coordinating with physicians, therapists, and support staff to ensure holistic healing.

You will mentor teammates, utilize advanced tools for documentation, and advance through a structured career ladder that supports clinical leadership and educational initiatives.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Pay Range

$35.53 - $52.85/hour
