CommonSpirit Health at Home is seeking a Registered Nurse for PRN home health visits. This role emphasizes meaningful patient connections, compassionate care, and the opportunity to witness the full arc of healing in one patient’s journey.

You will lead care plans, collaborate with a multidisciplinary team, educate families, and mentor other nurses and aides. A nurse license, CPR/BLS, and a valid driver’s license are required.

Qualifications

• Graduate of an accredited nursing program with an active RN license.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.

Pay Range

$35.53 - $52.85/hour
