Job summary

FellowshipLIFE's Home Care – Non-Medical Personal Care division in Freehold, NJ, seeks an RN Supervisor to lead the Home Care Visiting team and oversee field clinicians. You will ensure care plans are followed and coordinate with families to support residents’ well-being.

Requirements include NJ RN license, BLS, NJ driver’s license with reliable vehicle, and at least one year in a healthcare setting. On-call rotation participation is required for this full-time role.

Qualifications

• Strong communication and interpersonal skills required.

• Ability to work independently and in a multidisciplinary team.
• Proficiency with electronic medical records and related software.

Responsibilities

• Adhere to nursing board best practices and accreditation standards.
• Open communication with clients, families, and team members.
• Place care partners with clients as needed.
• Supervise CHHA staff for on-campus/off-campus clients.
• Make monthly follow-up calls/visits to ensure Care Plan adherence.
• Assess care effectiveness and complete 60-day skills assessments during visits.
