Job summary

Sentara Home Care is seeking a Home Health Coordinator- RN to coordinate and supervise nursing care for participants in our VA program. The role includes interdisciplinary collaboration, conducting assessments, and guiding health maintenance and disease prevention.

Pharmacologic administration may be required as part of patient care. The ideal candidate holds an Associate's Degree in Nursing, an active VA RN license, and BLS certification within 90 days.

Qualifications

• Associate's Degree in Nursing required.
• RN active in VA required.
• BLS certification required within 90 days.
• Minimum 1 year nursing experience required.
• Home Health experience strongly preferred.

Responsibilities

• Coordinate and supervise all nursing care for participants per program standards.
• Interact with interdisciplinary team meetings and hospital/therapy staff.
• Conduct initial and periodic assessments and document findings.
• Administer nursing care, advise on health maintenance and disease prevention.
• Administer medication as needed.
