Job summary
Sentara is seeking a Home Health Coordinator (RN) in Suffolk, VA, to coordinate and supervise nursing care for home health patients. The role includes assessments, interdisciplinary collaboration, and patient education. Requires an RN license active in Virginia and at least 1 year of nursing experience; home health experience is highly preferred.
The position is full-time with a day shift. Mileage reimbursement is offered for patient visits, and a comprehensive benefits package is provided,
Qualifications
- Associate's Degree in Nursing required.
- RN license active in VA; BLS required within 90 days of start.
- 1 year nursing experience required; home health experience highly preferred.
Responsibilities
- Coordinate and supervise nursing care for home health participants
- Conduct initial and periodic assessments; participate in interdisciplinary team meetings
- Administer nursing care and medications as needed; counsel on health maintenance and disease prevention
