Sentara Health Plans is seeking a Home Health Coordinator (RN) in Virginia Beach, VA to coordinate and supervise nursing care for home health patients. The role collaborates with an interdisciplinary team to assess patients and promote wellness, disease prevention, and proper medication management.

The ideal candidate holds an Associate's Degree in Nursing, Virginia RN licensure, and BLS certification within 90 days, with at least 1 year of nursing experience and a preference for home health

Formación

• Associate's Degree in Nursing required.
• RN active in VA required.
• BLS certification required within 90 days of hire.
• Minimum 1 year nursing experience required; home health experience preferred.

Responsabilidades

• Coordinate and supervise all nursing care for participants according to program standards.
• Interact with interdisciplinary team members and participate in team meetings.
• Perform initial and periodic assessments; administer nursing care as needed.
• Advise participants on health maintenance and disease prevention; manage medications when required.
