Job summary

Interim HealthCare Inc. invites applications for an RN Home Health Clinical Manager to oversee personnel and patient-centered care across Johnstown, Altoona, and State College. The role includes coordinating care, ensuring individualized plans, and leading QA initiatives while ensuring regulatory compliance.

Qualifications

Valid RN license in good standing.
Minimum of three (3) years of home health care experience.
Two (2) years of management experience.

Responsibilities

Oversee personnel and patient-centered care/services for the agency.
Coordinate patient care and referrals; ensure plans of care are updated.
Manage QA program (QAPI) including patient outcomes and experience.
Create a positive work environment to meet personal and business goals.
Ensure compliance with all laws, regulations, and Interim HealthCare policies.
Perform supervisory nurse visits and field home assessments; support in-office duties.
Assist with Clinical Orientation and onboarding training.
