Horizon Health Network in Clark County, Springfield, OH, is seeking a Registered Nurse to join our Home Health team. You will manage patient care from admission through discharge, perform OASIS assessments, and develop individualized care plans in collaboration with an interdisciplinary team.

Qualificações

• Current Ohio RN license.
• Experience in home health and OASIS assessments preferred but not required.
• CPR and/or BLS certification required
• Valid driver's license and reliable transportation

Responsabilidades

• Performs SOC, ROC, Recertification, Transfer, and Discharge OASIS assessments.
• Conducts comprehensive nursing assessments and develops individualized plans of care.
• Coordinates and manages patient care throughout the episode of care.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers and other team members.
• Monitors patient progress and adjusts care plans as needed.
• Completes OASIS documentation and records per Medicare, state and agency guidelines.
• Participates in quality improvement initiatives and regulatory compliance
