Job summary

Horizon Health Network is seeking a Registered Nurse (RN) for Home Health in the Mercer County area, including Celina, Ohio. The role covers patient care from admission to discharge, OASIS assessment, and development of individualized care plans.

You will work with an interdisciplinary team to provide autonomous, flexible home-based nursing care, focusing on patient comfort and recovery in the home setting.

Qualifications

• Current Ohio RN license required.
• Experience in home health and OASIS assessments preferred.
• CPR and/or BLS certification required.
• Valid driver's license and reliable transportation required.
• Strong assessment, communication, organizational and time-management skills.

Responsibilities

• Performs Start of Care (SOC), Resumption of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
• Conducts comprehensive nursing assessments and develops individualized plans of care.
• Coordinates and manages patient care throughout the episode of care.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers, and other team members.
• Monitors patient progress and adjusts care plans as needed.
• Completes OASIS documentation and clinical records per Medicare, state, and agency guidelines.
• Participates in quality improvements and regulatory compliance.
