Job summary

Your Health Organization in South Carolina is seeking a Registered Nurse to serve as a Nurse Case Manager for wound care patients across the Little River area. This full-time role requires home and facility visits, travel, and telemedicine coordination to support our patients.

As a key member of the care team, you will review medical records, enroll patients in Care Management, initiate Plans of Care, and collaborate with providers to ensure high-quality, compliant care.

Qualifications

• Must hold an RN license in good standing.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
• Strong written and verbal communication skills.
• Proficient in basic computer use and electronic health records.

Responsibilities

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Review medical records.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care if eligible.
• Capture diagnoses at highest specificity and ensure acceptance.
• Coordinate care and communicate with providers and families.
• Travel to patient homes and facilities as required.
