Job summary

VNA Care is hiring a registered nurse (RN) Case Manager to join our team for 24-32 hours per week. This role covers patient visits in the Arlington and Belmont areas, delivering compassionate home health care and coordinating care plans.

The ideal candidate holds a MA RN license, has at least 1 year of med/surg experience (acute care a plus), and is willing to travel within the designated territory. You will work with an interdisciplinary team to ensure high-quality patient outcomes and thorough

Qualifications

• Must hold MA RN license in good standing.
• Minimum of 1 year of med/surg experience; acute care beneficial.
• Willingness to travel within a designated territory.
• Home Health Care experience required.

Responsibilities

• Conduct home visits to assess patient needs and provide necessary care.
• Collaborate with patients, families and interdisciplinary team to develop care plans.
• Utilize Oasis E documentation, Time Management and EMR Home Care Home Base for records.
• Ensure accurate documentation of patient progress and treatment plans.
