Job summary

VNA Care in Massachusetts is hiring a Case Manager RN to work 24-32 hours per week, covering the Arlington and Belmont area. You will conduct home visits and coordinate patient care to ensure they receive the services they need to thrive.

Requirements include an MA RN license in good standing, at least 1 year of med/surg experience (acute care a plus), willingness to travel within the territory, and Home Health Care experience. Schedules are Mon-Fri, 8:00 am–2:00 pm or 8:00 am–4:30 pm.

Qualifications

• Willingness to travel within a designated territory
• Home Health Care experience

Responsibilities

• Conduct home visits to assess patient needs and provide necessary care
• Utilize Oasis E documentation, Time Management, and EMR(Home Care Home Base)
• Collaborate with patients, families, and interdisciplinary team to develop and implement care plans
• Ensure accurate documentation of patient progress and treatment plans
