Job summary

AccentCare in Austin Office is seeking an RN Case Manager for Home Health. You will manage patients' care plans from admission through discharge and coordinate a team to deliver quality home health services.

The role requires Texas RN licensure, at least 1 year of RN experience, and CPR certification. Schedule Monday–Friday 8am–5pm, with on-call duties; coverage area includes Bastrop, Del Valle, Lockhart, and Smithville. Salary range $91,000–$112,000.

Qualifications

• Must be a graduate from an approved school of professional nursing and currently licensed as a registered nurse in the state of operation.
• Minimum of one year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess the status of nursing needs and manage the patient's care plan.
• Complete initial patient assessment upon admission and collect history.
• Select nursing diagnoses and monitor medications for effectiveness and safety.
• Establish measurable goals aligned with diagnosis and inform physician of results.
• Implement nursing interventions within the Nurse Practice Act and scope of practice.
• Re-evaluate patient needs and adjust plans as required.
• Document assessments, progress, discharge notes, and required paperwork promptly.
• Inform physicians and staff of changes in patient condition and needs.
