Job summary

Qualifications
- Active DE RN license.
- Minimum 1 year of home health experience.
- Ability to travel to cases as assigned.
Responsibilities
- Follow prescribed care plans and patient needs.
- Make home visits to clients in designated territories.
- Administer medications, provide wound care and treatments.
- Monitor patients and report changes to supervisor.
- Document observations and care provided in EMR.
