Job summary

BAYADA at Bayhealth is seeking an Admissions or Case Manager Registered Nurse for a full-time or part-time role to serve adult patients in their homes. The office covers Sussex County, DE, focusing on Seaford, Milford, and Laurel areas.

The RN will work with a multidisciplinary team to provide rehabilitative and skilled nursing care so patients can remain at home. Responsibilities include care planning, visits, documentation, and coordination.

Qualifications

• Current license as a Registered Nurse in DE.
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience preferred but not required.
• Graduation from an accredited nursing program.

Responsibilities

• Follow a designated care plan for each patient.
• Make home visits to clients in designated geographic territories.
• Administer medications, provide wound care, treatments and procedures.
• Monitor client conditions and report changes to supervisor.
• Document physicians' orders and client assessments.
• Coordinate case management and care activities.
